STEP
1 You will need Adobe Acrobat Reader in order to open
and complete the application. If you already have Adobe Acrobat
Reader skip to step 2. If you do not have Adobe Acrobat Reader,
click on the following link and follow the instructions. Once Adobe
Acrobat is installed, proceed to step 2.

2 Click on the following link to open the credit application.
Once you have completed
the form, click on the submit button at the bottom, and the completed
form will be attached to an E:mail to MAC Financial Services, Inc. You
can include a message in the body of the E:mail, or just click send.
If you need to clear the
form, or start over, click the clear form button at the bottom of the
page.

3 Return
to MAC Financial Services, Inc. Web site.
Once you have completed and
submitted the form, click on the return button at the bottom of the
form. This will return you to the MAC Financial Services, Inc. Home
page.
